A written request for access to a public record shall be submitted on the required form and submitted to the Open Records Officer either in person, by mail, or to a designated facsimile machine or designated e-mail address.
Each request must include the following information:
- Date of request
- Medium in which the record is requested (i.e. E-mail, U.S. Mail, Fax, In-Person)
- Name of requestor
- Address of requestor
- Description of specific record requested in sufficient detail
